      One piece with the rope? Hmmm....probably mostly hand carved. You could cut the circle with a router or Rotozip and a circle cutter attachment, then remove the material between the rope and the inner points of the star with a scroll or jigsaw, depending on the size of the star. After that, it looks to be mostly chisel and knife work. A Dremel might come in handy for the rope detail and sanding.
      As far as drawing a pattern, just draw a circle with a compass, then use a protractor to mark 5 equal points 72 degrees apart. Split the difference for your inner points and you're ready to go.
