Why does my table saw belt keep slipping? I've tightened the tension but it keeps slipping off. Any ideas?

The belt can be slipping due to a variety of reasons.

If the tension is right then the alignment is most likely off.

I have a Ridgid TS3660 table saw. Tension and alignment both seem ok. I can get about 6-10 cuts, then it lossens right up.
